
Annette is our Cancer Care Coordinator, here at Maritime Health Partnership. She is most passionate about helping and supporting our cancer patients from the very start of diagnosis and throughout their journey .
More people than ever are living with and beyond cancer. Receiving care that is tailored to an individuals needs can have a significant impact on their experience and quality of life.
Our Cancer Care Coordinator provides a service to support people living with cancer, enabling them to take an active role in the way their care is planned and delivered, with care tailored around the things that matter most to them.
How can a Cancer Care Coordinator Help Me?
Annette, will spend time with you to understand your concerns, and those of your family, to identify the support available, either at one of our practices or in the community. Some of the support you can expect includes:
•Support from start of diagnosis and throughout your journey
•A trusted and confidential point of contact
•Referrals to appropriate services that may help you and your family
•Providing access to information about your diagnosis, procedures and treatment
•Assisting your Specialist, General Practitioner and other Health Professionals in coordinating your care
•Ensuring you are well supported by all relevant members of the treatment team e.g. Social Worker, Physiotherapist etc.
•Providing information on support groups and organisations, including but not limited to; Macmillan, welfare and mental health services
